In most cases, I will need to see a valid ID from the signers. This can be a non-expired passport, driver’s license, or government-issued ID card, or another form of government-issued ID that contains a signature and a photograph, which I find satisfactory.

If neither of the above criteria can be met, verification on oath or affirmation of a credible witness personally appearing before me and whom I can identify by one of the forms above is an acceptable alternative.

If you’re physically unable to sign the document yourself, you may direct another person – other than the notary – to sign your name on the record by proxy. This also requires two additional signatures of witnesses, one of whom can be the person signing by proxy, as long as both witnesses are unaffected by the record being signed.

There are some instances that require a signature to be made under oath and must be witnessed by the notary, while others only require the notary to verify the signatures on the document. If you are in doubt which you need to do reach out to me before signing your documents or simply wait until the appointment to sign your documents.

A notary may refuse to sign documents if they are not satisfied that:

(1) the person executing the record is competent or has the capacity to execute the record;
(2) the person’s signature is knowingly and voluntarily made;
(3) or for other reasons, unless prohibited by law other than KRS 423.300 to 423.455.

Payments are due upon arrival, prior to notarizing your documents. The fee schedule below describes the estimated charges you may incur.

Fee Schedule:
• $5 per signature, stamp or document
• $5 travel fee if within Fayette County
• $10 travel fee if in a County surrounding Fayette
• $25 minimum travel fee for other KY Counties
• $5 minimum convenience fee for appointments outside of normal business hours, special appointments, or during holidays
